Nepenthes gracilis hybrid?
This plant came from Europe and lacks any location data. It was imported to the USA with the label "N. anamensis", but even the folks who imported them doubted the name was correct, but the name was retained since that is how they were distributed. Information brought to light by Nepenthes of Thailand web site also indicates this plant is probably not Nepenthes anamensis.
I think it could be N. gracilis * N. kampotiana. I suspect this as the leaves are nearly a match for the leaves of N. kampotiana; which have narrow, sessile leaves which are less decurrent than those of N. gracilis which have leaves which are completely sessile, and extremely decurrent. Also, the shape of the pitchers are very close to those of N. gracilis and share some traits with N. kampotiana (Cambodia), like the ventricosa (or 'belly' as the Thai call it) lower half of the lower pitchers.
